The Greenville County Coroner has confirmed that Heroes and Nashville actress Hayden Panettiere died from the toxic effects of fentanyl Xanax and three other drugs in an accidental death ruling.

The cause of death of Hayden Panettiere, the actress beloved by millions for her roles in Heroes and Nashville, has been officially confirmed as accidental drug toxicity.
The Greenville County Coroner’s Office confirmed to Variety that Panettiere’s death was caused by the toxic combination of fentanyl, 4-ANPP, Alprazolam, Methocarbamol and Quetiapine. The manner of death has been ruled an accident. There were no signs of trauma.
Panettiere was 36 years old. She died on August 16 in Greenville, South Carolina.
What Happened
At approximately 1:51 PM on August 16 a 911 call was received regarding a cardiac arrest at a residence where Panettiere had been temporarily staying. An acquaintance made the call. First responders discovered her unconscious and initiated life support. She was pronounced dead at 2:32 PM.
The Greenville Police Department confirmed no foul play was suspected.
The substances found in her system tell their own devastating story. Fentanyl is a synthetic opioid up to 100 times more potent than morphine. 4-ANPP is a chemical intermediate used in the illegal manufacture of fentanyl. Alprazolam is better known as Xanax. Methocarbamol is a prescription muscle relaxant. Quetiapine is an antipsychotic used to treat bipolar disorder, schizophrenia and major depressive disorder.
The combination of a powerful opioid with sedatives and a muscle relaxant is frequently fatal.
A Career That Began at Five
Hayden Panettiere began her acting career at age five on the soap operas One Life to Live and Guiding Light. In 2000 she landed a breakout role in the sports drama Remember the Titans alongside Denzel Washington. The following year she was cast as cheerleader Claire Bennet in NBC’s superhero series Heroes, the role that made her a household name.
She starred in the musical drama Nashville from 2012 to 2018, earning two Golden Globe nominations. She played fan-favourite Kirby in Scream 4 in 2011 and Scream VI in 2023. Her screen credits also included Ice Princess and Bring It On: All or Nothing.
Her Own Words
Panettiere had been open about her struggles with alcohol and drugs for years. She stepped away from the industry in 2016 before returning in 2023 with Scream VI. In May 2026 she published a memoir titled This Is Me: A Reckoning, in which she wrote candidly about those struggles and the impact of growing up in Hollywood.
She left that book as her final word. The coroner has now provided the facts behind it.
