To apply for a NADRA CNIC name change, the most important document is a computerised birth certificate issued by the Union Council (UC) or Cantonment Board showing your updated name. If this is not available, applicants can submit an affidavit form specified by NADRA — Form C1 for B-Form and Form C2 for CNIC. In case the name change is due to a change of religion, a certificate issued by Dar-ul-Ifta is also required. Make sure to carry your current CNIC along with these documents.

At the NADRA registration centre, the first step is to obtain a token. When your turn comes, biometric verification will be completed, followed by data entry by a NADRA officer. Your fingerprints and photo will be captured. And you will be given a printout to confirm that the details are correct. The office in-charge will then approve the application and may ask basic family-related questions. NADRA has clarified that there is no requirement to get the form attested for CNIC amendments.
Fee payment and collection
After approval, you will receive an SMS confirmation. The fee structure is: Normal category – Rs 750 (30 working days). Urgent – Rs 1,500 (15 working days), and Executive – Rs 2,500 (7 working days). Once processing is complete, you can collect your new CNIC from the same NADRA office where you submitted your application.
